Coaching Disclosure & Authorization

Operation HOPE provides free coaching services and financial management information to individuals and small business owners. Operation HOPE will NEVER ask you for money nor will we pay your debts for you. Our coaching services are intended to be used only as general economic and financial resources. Please be aware that our information and coaching services are not a substitute for the economic, financial and other professional advice and services provided by banks, other regulated financial institutions and licensed financial professionals.

Operation HOPE does not provide investment, legal or tax advice. Accordingly, you should contact a lawyer, accountant, bank or other financial institution before making any financial decision or entering into a financial transaction. Operation HOPE is not and will not be responsible for any liability associated with your financial decisions

and transactions.

• I acknowledge that I have received, read, understood, and agree to the terms of Operation

HOPE’s Privacy Statement

• I understand that Operation HOPE is a self-help organization whose objective is to educate and provide guidance and assistance only.
• I hereby authorize Operation HOPE and its employees and volunteers to collect personal information relevant to provide financial guidance or counseling services, which may include obtaining a copy of my personal credit report as needed during my participation in the program (if authorized to run credit report)
• I further authorize Operation HOPE and its employees and volunteers to share and disclose relevant personal information to other assistance agencies and/or organizations for the purposes of providing financial services
• I authorize Operation HOPE to obtain the information deemed necessary regarding my financial condition and credit worthiness. This includes financial statements, employment documents, savings and other depository accounts, past and present consumer credit records, copies of income tax returns, and/or outstanding credit and payment history (mortgage, auto, home improvement, charge / credit cards, credit unions, ). Operation HOPE, or its bank partner, is further authorized to use a copy of this form, to obtain the before mentioned; update financial statements and re-verification of information.
• I acknowledge Operation HOPE financial services providers consist of employees or volunteers who are not licensed or certified by any governing body and, therefore, I should always consult a licensed financial professional for advice prior to making any important financial decisions
• I release Operation HOPE and its employees and volunteers from any liability associated with the information or counseling services provided
• I hereby agree to hold Operation HOPE , its employees and volunteers harmless from any claim, suit, action, or demand made by any creditors, agencies, companies or organizations to which Operation HOPE may refer, or any other entity or person which in any manner may arise from any action or inaction taken by any entity or person, in connection with any services rendered by Operation HOPE and its employees, volunteers and/or agents.
• I understand that Operation HOPE provides consumer credit information, coaching, guidance and assistance after which I may have access to an action plan (goals & homework) consisting of recommendations for handling my finances, possibly including referrals to other agencies as appropriate;
• I understand that Operation HOPE provides information and education on numerous loan products, credit products, and housing programs and I further understand that I am under no obligation to use any products or services of Operation HOPE’s industry partners, including those of financial institutions offering loan products or realtor services, (see list of partners below). Enrolling in other Operation HOPE programs or services is not a requirement to receive housing coaching.
• I hereby agree that, in the event Operation HOPE assists me in purchasing a home, securing a loan, modifying my loan or obtaining an acceptable workout plan, I will provide a copy of said outcome to Operation HOPE for its secure files for program validation.

DISCLOSURE OF OPERATION HOPE PARTNERS

Homeownership Program

As a Homeownership Coaching client, I understand that:

• I am under no obligation to use any products or services of Operation HOPE’s partners,

including those of financial institutions offering loan products.

• I will be required to provide documentation deemed necessary for my pre-qualification including statements of my savings depository account(s).

Credit Authorization Form

To assist Operation HOPE in achieving my objectives, I/We hereby authorize Operation HOPE to obtain the information it deems necessary regarding my/our financial condition and credit worthiness. Operation HOPE or its successor in interest is further authorized to use a Photostat copy of this form in order to obtain the aforementioned information.

You may be asked to show your identification card, if you request a credit report.

Financial Documentation

As a part of HOPE coaching programs, please provide official documentation of your income, savings and debt (where applicable). Examples of documentation are listed below to provide to your HOPE financial well-being coach so they may create an accurate financial assessment, as a part of your financial planning.

Income Statements

This is official documentation of your bank statements or paycheck stubs, for at least two pay periods.

Savings and Accounts Statements

This is official documentation of savings, bank accounts and retirement accounts. Please mark through your account number, as we do not need that information – if applicable.

Conflict of Interest

As a HUD approved Counseling Agency Operation HOPE is required to fully disclose all actual and potential conflicts of interest.

Operation HOPE prohibits the solicitation and acceptance of gifts or gratuities by employees, volunteers, and agents for their personal benefit.

A conflict of interest would arise if the director, employee, volunteer or agent of a participating agency or the spouse, child or business partner of any individual holding these positions of trust or any organization in which these persons serve as an employee (other than Operation HOPE), or with future employment, has a direct interest in the client by virtue of their role. Some

examples include, as the client’s landlord, real estate agent, broker, creditor, mortgage broker,

loan originator, insurance broker, and title officer.

1. Violation of this code will be subject to disciplinary action which could include immediate termination.
2. This code is applicable to all Operation HOPE programs and transactions.
